Fix some Html PHP code on two basic pages. I must have this completed by Sunday evening. I have two online forms that I need some assistance in debugging. I will send you the code I have and you can run it from your site. Basically the two forms I will refer to as Crim form and Agreement form.
Crim form
The user fills in the form and the captcha and clicks submit/email and the form data should be emailed to the address in the code. Problem is that I never get the email. Possible something to do with SMTP authentication missing, but it seemed to work for awhile just using the basis PHP email function without specifying any smtp parameters. I suggest you remark out the captcha while testing so as not to have to bother filling it in each time. You should edit the email address to point to an email address you can access so you can verify it is working. Once done I will edit email back to my address and test. I use a standard pop email account on my domain which is hosted at network solutions.
The other item I need fixed is that the last two lines of the form have a longer description than I currently have on the form and that needs to be fixed so that the forms entry descriptor wraps taking two line so as not to take too much width. The user fills in the form and clicks print. The agreement gets filled out and printed to their local printer. The problem is that the print out is missing one page of text that should get printed at the same time as the agreement on its own separate page before the agreement page. These instructions should not be displayed anywhere else but on the print out. The instruction consist of a few lines of static text and no images and I will include it in the code I send you if you are hired.

2 - Crim Form adjust so description dual line.
3 - Agreement Form add instruction page to be printed.

Do not use any browser pop up windows or anything client side that would require end user to have specific browser configuration settings. Email or call me if have questions as soon as possible so we can save time and not waste time coding and going down the wrong path. Include comments in code. I can program but am not very familiar with PHP and I would like you to explain some details on the code. As I am under the gun to get this complete I would prefer a local coder that is near the same time zone so we can talk on the phone to help speed up the process. I would think an experienced PHP coder could get this done in 1.5 hours. Add time to review the project and submit the code and answer some questions call it around 2 hours.
