Usability Rreport - Survey Creation and output to report

I am looking to provide some Usability reporting services to my clients and would like a 10 step survey to be created that will output to a mysql datatabse, admin tool and pdf as a report. Here is an example report : [login to view URL] Currently the data is inputted into a .xls file and then graphs are created using this data in a cumbersome way. So the end goal will be to provide clients with a pdf file that will show results from different usability methodologies in one document, outlining the strenght and weakness of the products. Initially I would like one report to be built (an report output) and if the agreed developer does a good job then more reports will be built and the relationship will continue with good value. I expect the delivery of this project to be within 30 days, and on finishing all the data/code on the project will belong to myself. An intial payment will be made and deposited in Escrow, and then full payment will be made on completion of the project immediately. Please only serious bidders here, - companies who would like a long term relationship if possible, and are willing to communciate regualrly through im software (skype/yahoo) If an outstanding project is delivered, an additional bonus will also be added on top of the intitial agreed project cost.
