Javascript Form Validator in Japanese HTML page

Below is the HTML for top of the page in question: ---- ---- TASK: I attempted to use a javascript form field validator but have been unsuccessful. I simply need to have some of the order form fields required. Ideally, the 'req' comments should be in Japanese, but if not that is acceptable. ## Deliverables Rent A Coder requirements notice: As originally posted, this bid request does not have complete details.
