need new form to work like old one

there is a current working form for purchasing a free trial. There is a working form with a credit card section. a new form was developed. I want this new form to be able to submit like the old one. I can give you all files but not access to the FTP. You will have to work locally to fix this.

My maximum budget for this is $15 maximum.
