document signing wordpress plugin - repost

Hi, I need a plugin creating for document signing in wordpress. In the backend the wordpress site owner can paste terms and conditions, terms of use etc into a box the front end user has to digitally sign the document by typing thier name, initials and date and check I agree to this check box and email address box when they have done this they hit submit, it then gets logged in the wordpress backend with the ip address and users internet setting i.e. browser choice etc logged to the document and saved in the backend and a copy is automatically emailed to the clients email that they used in the signup. It must be able to be shotcoded into a page i.e. insert [document-signing] into a post or page I have an example plugin I can provide to the winning bidder but this needs to be a custom plugin created and completely unique but it is actually a very basic plugin. so I know you have read and understood this, start your bid with the word documents
