Survey-application to run on Java on mobile phone plateform

Requirement [login to view URL] application should be able to read a text-file or xml with a script describing a simple questionnaire. The questionnaire should be displayed on the java enabled phone/PDA, so that the user can then fill-out the questions. 2. Text Box - Open Ended. e. Date (MMM/DD/YYYY Drop Down). f. Branch Logic. 5. Ability to Upload Images into Questions and send (Only in case of uploading to website). 6. Ability to get GPS coordinates form inbuilt GPS chip and upload into question. Storage Options must be available a. To store data as csv/ txt appending the old data on the mobile itself. b. To send collected data as sms. Important : One sms for one form c. To transfer the collected data to a webpage using GPRS. Data delivery 1. Possibility should be there to upload collected to website (receiving pages can be coded in ASP or PHP with access or mysql database ) after uploading each form successfully then confirmation should be displayed on mobile. Eg Record has successfully been uploaded or anything on similar lines. 2. There should be system of Login ID & Password. And user id must get appended to the record uploaded/saved with date and time. 3. Output of each form is able to be sending as SMS. Support common resolutions of PDA 240x160, 320x240, 200X400, and 240X400.
