Create a prototype website + app based on our detailled requirements including visuals

Beginning of next year we will be promoting a new concept that contains both a website for health insurance companies and an iOs-app for customers that visited a doctor. So the idea involves: - Health Insurance Companies (HIC) - Customers that went to a doctor (CUST) - Companies that give health, i.e. a dentist) (CARE) The basic flow is that a CUST visits a CARE. The CARE helps the CUST and send an invoice (DECL) directly to the HIC the CUST is connected to. The HIC checks the DECL and pays the CARE. The problem with this flow is that the CUST never sees the amount and number if DECL the CARE sends to the HIC. This leads to massive fraud as CARE can send DECL without the CUST even visited. What we want to do, is give the CUST an app where he can see and judge every DECL that the CARE has send for him to the HIC. Also the HIC should get a website where he can check all the results from all his customers. We designed almost 80% of the app and 80% of the website, but for now we need a working prototype where we can easily manage lists of HIC, CUST and CARE and where we can show that a DECL from CARE will pop-up in the CUST app and that the reply of the CUST pops up In the HIC website. The working prototype will be used to pitch our idea to several HIC. What we need is quote for a working product: The product should be a working website on [login to view URL] (which we already own) and a working iOs app. The price must be including install + set op of de SQL DB on our domain. Basic functions are: - desktop screen to manage list of HIC (easy editing of the SQL table) - desktop screen to manage list of CUST (same) - desktop screen to manage list of CARE (same) - desktop screen to enter a DECL (same) - push notification in App (that a DECL is ready) - list of DECL in App - Details of DECL in App to judge it - hamburger menu in App - screen in App to connect a HIC to a CUST (using a 5 digit code) - function to generate the 5 digit code that is used to uniquely connect a HIC to a CUST - screen in App to enter some personal details (mail, phonenumber etc) - screen in App to show all HIC the CUST is connected to - Message in App when DECL was judged - desktop screen with Dashboard (top 5 CUST, CARE and DECL) - desktop screen with All DECL (sortabel, filterable) - desktop screen with details per DECL - desktop screen with All CUST - desktop screen with details per CUST - desktop screen with all CARE - desktop screen with details per CARE
