Setup namebadge template

I need someone to setup a template for me to print name badges for a conference. I need the logo for our organization which is CISPROH in the top centered (see attachment). I need the logo for Listerine in the bottom in the Center and above it it should say "Sponsored by" (see logo below). In the center there should be a text box for me to enter the names. there is 6 badges per badge. I have attached the bade template. The turnaround must be pretty fast. So essentially the template should be setup with the CISPROH logo in the top in the center, the listerine logo in the bottom in the center and above it it should say in small writing Sponsored by AND should have a text box for me in the center to easily place the participants names.
