As a psychology practice, we often get clients to fill in questionnaire data in Google Forms. This raw data is then converted into Google Sheets. From here, we have set up some formulas so this raw data is converted into scorable material that we can use to figure out cut-offs and normative results.
“How happy are you?
Very happy | Happy | Average | Not Happy | Very Unhappy |”
Once we receive the answer for such a this is entered into a ‘Form response’ sheet in Google Forms. Next, we have created a separate sheet within the same workbook which is basically array formulas: e.g. =arrayformula( if( row('Form Responses 1'!N1:N) = 1, 'Form Responses 1'!N1, 'Form Responses 1'!N1:N ) )
From here, we have created another sheet within the same workbook (see below):
This scoring is then added up and made into a total score or a cut-off score etc.
We have been using an add-on called ‘Document Studio’ which can grab variables from the ‘Form Responses 1’ sheet and put it into a PDF file which can then be directly sent to our relevant clinicians. While the raw scores often work, the scoring does not (see document attached for screenshots of what we actually receive - it is circled).
The data from the ‘Form responses 1’ worksheet comes through with the variables but the formula data that we have used to score, does not.
We have already tried to double check the variables multiple times – the variables are being picked up (see screenshot in attached document). However, they are just not being entered into the template as we want.
You can find our template that we use in Document studio here:
