Design of a basic electronic medical record, for eg using Microsoft access. This is for the purpose of a consulting only optometry clinic (no sales)
I need:
A main database with all patients. The main page should include (in the form of rows and columns) :. Exam date (auto), Patients first and Last names, date of birth, medicare number, expiry date, verify (yes/no options), refer (yes/no/urgent options) contact name, phone number, address.
I need to click on an individual new row to create a new patient record. The information in the main database should autofill into the patient record, and vice versa.
The patient record consists of multiple pages in the form of tabs
The main page is titled:General info
This page needs to include all the info as the main database , in addition to the following blank fields: presenting complaint, previous ocular history, general health, family ocular history, unaided VA, Aided VA, pupils, excursions, cover test, comments. Each one of these fields should have an autofill option of predetermined text, and/or a type in option.
The next page or tab is called:refraction. This will include the following fill in or autofill fields: subjective refraction: RE/ LE, retinoscopy RE /LE, Final Refraction.
The next page is called binocular vision assessment : this includes: NPA, NPC, phoria, facilities, stereopsis, colour vision. More detail will be giving about this page.
The final page is called ocular health and management : internal assessment, external assessment, final management and advice.
All record page fields should have autofill and type in options.
A prompt at the end of the exam should ask if a patient referral is required. If yes, a referral needs to be generated using the data from this record. The database should show that a referral has been done on the main database page.
