Fill in an Excel Spreadsheet with Data from a business directory as scanned PDF

I have a PDF file with approximately 15 records per page, that is about 250 pages long.
The document contains approximately 4000 records to type.
The resulting spreadsheet needs to contain the following fields completed:
Firstname, Middle Initial, Last Name, Company Name, Address, City, State, Zip, Phone, Fax, E mail
There may be more than one contact name at a particular company, in which case you will be paid on a per-record basis for the total resulting records transcribed.
Please provide a per-record quote, as well as a quote for all 4000 records transcribed.
Understandably, we don't know if it is exactly 4000 records, so if there are more than 4000 records, you will be paid an average per-record overage to be fair with you.
