I have 4 documents (questionnaires and forms) for a doctor client of mine that need to be completely restyled.
- Fonts need to match
- Layouts need to be cleaned up
- Lines on questionnaires need to alternate background color to stand out.
All of these documents need to be converted into PDFs that can be filled out online. In other words, a patient need to be able to fill them out using their browser then, save, print or email.
All of these documents need room at the top for the logo (Final logo design needs to be approved).
Attached are all 4 documents. I would like these done quickly if possible.
