fix one ASP contact page

I have an ASP contact page on a website that sends a customer contact email to me. It worked fine for many years. Then, it recently stopped. I hired someone from freelancer who fixed the page, but he changed the format so that the "from" email address is my own email instead of the address of the customer. SInce he did this, i have been deluged with spam and i think the server may have been blacklisted. (SPam blockers cannot block my own email address).
So, i need someone to fix this one page so that the emails it sends me are from the actual customer. Hopefully this will stop the spam (I never had one form-related spam email in 18 years before this).
This is just a job to fix one page and have it work right, hopefully stopping spam.
