An Application on AccessDB using Access forms

Skills : Access 2007

I have access db which is attached in this requirement.

1) Database doesn't have Primary Key, Identity value settings.

2) Database doesn't have forms to enter / pass the values to the database.

Expected output

1) Set the Identity Column, Make sure the Primary columns and Table relationships. DONT WORRY ABOUT THE SAMPLE DATA.

2) We need forms which is developed ONLY on Access 2007. Forms will do data entry operation. Forms should have good look and feel. Best User Interface.

3) I need two releases. Release 1 :- Form is separate from DB. Lets say, i can keep the form on my Desktop and double click and do the data entry. DB is in my C:\DropBox ( Dropbox is the cloud)

Release 2: Give me the form and DB together.

Very simple project. I am not a Good UI Developer. So , i am depending on experts to help me.

DB has 5 Tables

1) Users - Who is the log in user / Who entered the data

2) Casemanagers - Managers who are working for certain Organizations.

IsActive column will tell whether a Casemanager is still an employee with an organization or not. One Casemanager can work as a Freelancer without having an Organization or can work for more than one organization at a time.

3) Organization - List of Organizations available in the Market. Organizations have IsActive column. If they are inactive, value would be 0.

4) Communication :- This table is mainly for Users who talk to the casemanagers / organizations. Communication type can be an Email, Telephone Call, Fax etc ..!!

5) Notes :- This table is entered by Users. Users after communicating to CaseManagers / Organizations, they will enter the details as Notes. Every organization will have a contact person (Casemanagers). Contact date field will tell, when the user contacted.

Please show all these data's in a Grid table. Add / Edit / Delete feature for Casemanagers, Organizations, Notes is mandatory !!!
