A paper based form, converted to web based form for my website

I have an enrolment form currently as a word document that i want to be made into an online form. Online payments will not be part of the form, i only want the person completing the form to choose their method of payment. Part of the form is optional and when a person completed the form a checks a "NO" option for the AVETMISS section of the form, the form collapses hiding the questions. When a person submits the form it will be sent to me as an email.
