convert 24-question survey to html and css

This is a small project. -I need 24 survey questions converted to one or two HTML pages, and an accompanying CSS. The questions are contained in these three JPGS. The questions should be centered on the page, with ample left and right margins. The responses, which require, should mimic as closely as possible the manual process, which requires making a pencil mark along a horizontal rule (see the JPGS). -At the end of the questions there should be a submit button labeled “Done”. -There should php code, or lavascript, to check that all 24 questions have been answered. -The results to the questions should be saved in an php array
