Convert a Template to a Website

Looking for someone to convert this PDF into a 1 page HTML with inline CSS. The document has to be designed to fit 816px X 1024px. I need some changes to the PDF. in the html I need the drive prime logo across the bottom as a footer and I need the prime protection accross the top as a header. I attached a headerfooterexample pic. I need this project rather quick. this is used to display on a PC or IPad but also printed out as a brochure. So its real important the size is no larger than 816*1024.
The PDF being coded is only page 2. Also, the background needs to be white to avoid high ink charges if customers decide to print the brochure.
