Landing Page PSD (CS5 Compatible) for Child Support Payment Enforcement Lawyer

I need a one-page landing page design done ASAP, the faster the more you get paid on top of your bid BUT ... do not compromise QUALITY. I select bidders who take initiative. 1.) I'm looking for a design using [login to view URL] as a reference. 2.) I need '2' variations of the landing page. One with a contact form that looks like the reference sites contact form and another using the MadLib style contact form on this site http://smittyspoolservice.com. Contact form details will be identical to [login to view URL] 3.) Like [login to view URL] , I want the design to include '3' images that can be scrolled by. Don't include the '3' buttons below the sliding images, since I won't have those options and our images will be automatically scrolled. So you can just show generic 'law' related images, maybe show a mother and child, a nice looking lawyer etc, images need to be royalty free, I will pay extra to ensure this, if I find the image on Google I will not accept the project and you will receive a bad rating (as I'll get sued... and we don't want that). 4.) A blue like header bar just like [login to view URL], and the colors used on that site is preferable. Although client does not have full website yet, so for now just put 'Home' and 'Testimonials' in the bar. I attached an image and circled the features from the [login to view URL] page that I believe my client wants for this first attempt. The more variations of this design and 'unique suggestion' designs you can offer with your bid the better. 5.) Slogan should allow for text to change, for now main title should say "Child Support Payment Enforcement Attorney", 2nd phrase, "If I don't collect, you don't pay!" 6.)
