I have a word Document that I need to turn into a form. I have people that want to give me information and they want to type it into a from, then save it in Word and send it to me via email
So I need you to take this Word document and make it into a form where there are drop downs for the "Yes" and "No" Areas, Radial buttons for choose areas, and other areas where they can type in Text. Then save it and email it me in Microsoft word.
Or if you have a suggestion of excel or something else. I've included the document for your review to look at. I would be nice for you to spruce it up a little to make it look more professional:)
