C++ to C# Application

I have a medical billing application written in C++. I want it to rewrite in C# using VS2008,SQL server 2005, DevExpress .This would be a windows application(Not a web application). I could not upload the source code for exisitng application and also the exe. Now please look into the pdf file and come out with the database design and an application to access the database and also send claims electronically in the format described in the pdf file attached to the bid request. Please ask questions if you have any. I can show the application through gotomypc to few selected coders, but I do not know if that is possible in next couple of days. however please read the pdf file and give an estimate to create the database and the application.
