I wish to redevelop the transactional portion of www.childrenslifeinsurance.com. At present it runs to more than 10 pages (see attached zip file). I wish to reduce that to 4 pages while keeping all existing error checking. Rate quotes will be looked up based on a rate table stored in the underlying database. The each field of each application, including incomplete applications, must be stored in a database which includes time stamps for each submission. I will also need the site to automatically populate a fillable pdf (which will be provided) application which will also be stored in the database. I will provide a detailed spec listing required elements for each screen. All of the elements currently presented will need to be included in the new site. In addition, I wish to present the completed application in pdf form in a viewer on the page instead of as a separate window. Otherwise, I wish the current look and feel as well as the full set of links to other parts of the site to be preserved. The web site must be compatible with all major browsers including iOS Safari (no flash). Prior to coding, I will require a set of jpg mock-ups.
