My (very cheap) bid is for a Windows program that I'll write to do what you want. You'll be able to input the ZIP codes (which can be a list of all the ZIP codes in the USA, of course, that I''l provide) and a list of categories and then let it run and extract what you're interested in a CSV file. That way, you you'll be able to crawl this info again at any point in the future, for any categories.

I would like to offer a rate that is quantity-based: $2/1,000 lines or rows of contact details. I can scrape the Yellow Pages for your categories; however, I am unable to provide you with the contact details that are not found in the Yellow Pages; such as the email address.
