Create PHP Form Mailer

We have a Virtual Dedicated Server hosted on [login to view URL] We require you to create a PHP form mailer that will email the contents of the form to our email and store each submission in our MySQL database You must research how to do this on [login to view URL] Our HTML form has been created by us already. We require you to modify the site in our environment. We require a confirmation page. The page will be named "[login to view URL]" We require you to place our Google Analytics tags on the confirmation page. Once completed, we require all of the modified files.
