Display Data from DB/SQL - From Order Form

I'm looking for someone who can do a fairly easy task. It involves displaying data that has been submitted to database to the front-end for logged in users. The only catch is that it has to be filtered so that the logged-in user is able to view data that he/she entered. It can be displayed in a module or component...i don't mind either. The form an the database are already set-up. Your work will be to only write a SQL Query that displays the data for the specific user.
