Custom Magento Form to be Emailed

Hello All, I'm looking for a programmer who knows the inter workings of Magento 1.7.x well and PHP. I need two simple forms that you can find here: Form 1: [login to view URL] Form 2: [login to view URL] I need them coded so they show up on the Magento site that you can find here [login to view URL] for example (Blank page but this form will be here) both forms will have some basic form validation requiring a couple fields on each. Once a user completes the form I need it to be emailed. These form do not have to be stored in the database. If you have experience doing something like this with Magento please apply.
