Customize BreezingForms PDF Output

Need to customize the PDF output in BreezingForms component on this joomla site: [login to view URL] (just click through on "next" to see the form on the frontend.). The form's field sequences in the pdf output must be kept. So, in the exported pdf, instead of: Account Number Date of Birth (dd/mm/yyyy) First Name Gender Home Address Home Phone/Cell Number Last Name etc... I need: TRN (format:000-000-000) Account Number First Name Last Name Gender Date of Birth (dd/mm/yyyy) Contact Number Home Phone/Cell Number Home Address (Line 1) Home Address (Line 2) Monthly Salary Office Address/Dept Name, Address & Contact of Nearest relative Amount Being Requested (and so on...) Please see the attached PDF representing the layout I would like to achieve.
