Clone look & feel and functionality of a CSS/HTML/PHP form

We have on our site three forms created through www.jotform.com. However, we would like to remove this dependency. For that we would like someone to copy/clone the look & feel of the forms and write the functionality of our the "submit" buttons in PHP/JavaScript. The forms are: 1. A purchase form that is linked to paypal. 2. A thank you form. 3. A contact us form. Please look at how jotforms forms look like before applying for this project. We expect the forms to work smoothly and look the same on ALL popular browsers!
