Adding an email link to a php page

Hello
Is it possible to add a link to the bottom of some youtube embed videos where a user can click the button, enter an email and have a link to the said video emailed. For example if you go to [login to view URL], you will see an "Email Link" under the first video.
When clicked I would like:
1- Ask for the recipients email
2- Sender email to be some email
3- Subject to be "A video for dssurgery"
4- Some minor text at the bottom of the email.
5- Send a simple html email from the server and not client with a link to the video.
Is it possible to have the link open the video on the same page in a lightbox as it does now when you click on a video, or we can only send the link to the YouTube's website.
