ASP Form Error resolution/improvements

The two existing forms in question: 1. [login to view URL] 2. [login to view URL] What we need is: Diagnosing of two errors occurring with these forms. In one case, attachments that should be transmitted along with the form in an email are not working. In another, users are getting an error message when hitting "submit" in step 7 of completion. Once errors are resolved then additional work is needed to implement form improvements (ex. adding rollovers that "explain" a question), make wording changes, test Mozilla compatibility and build a french version of each using translated text that we would supply. ## Deliverables 1.
