Convert ASPX page from Dropdown to buttons and collect & post contact info. More work to follow.

**GOAL:** I'm looking for an ASPX programmer for small ASPX tasks that come up occasionally. This is a sample project. More work to follow. (In the past, I've done all of our ASP coding. I'd like to outsource those occassional tasks (especially since we're using ASP.net) **TASKS** Convert the attached .aspx page from an input by selecting from a Drop Down List to buttons. There are 4 choices in the List. I want, instead, to have 4 buttons ,one each for list item. **So the user will click on the button and that will SUBMIT their choice**. (Currently they have to click the drop down, click to select, then click submit). Put those 4 submit buttons into a 4 cell Table: (I'm not sure of the HTML here: it may require 4 different forms, one for each Submit button). Would be nice to do it as simply as possible (with one form) Patient -Basic Therapis-Basic Patient-Full Therapist-Full I will then add a bit of text to it to explain each option. **CHANCE TO DEMONSTRATE YOUR SKILL in 5 minutes.** I've included in the attachment, *[login to view URL]* That code could have been written much better: more efficiently, with less code, less effort. AND it would be easier to update if the name of any of the Install files (that it provides links to ) changed. You can take a look at that file and see if you can spot some improvements and post them. ## Deliverables **Additional Task** added 8/20/2012: **add a form field for user's phone # and email that with full name and email address.** So, when the user selects a Subscription Plan (clicking the right button) we'll submit the form info via email. 1. Add form field for Phone Number, Comments/Questions and "subscribe to newsletter", using the field names and submission info from the following page: [login to view URL] NOTE: you may need to disable cookies (or just run your browser in "anonymous" or incognito (for Chrome) mode b/c after you view it once it will not show you that page again, it'll just go directly to the downloads. (We do this so folks don't have to fill that out EVERY TIME. 2. set the Comments/ Questions? field to "Subscribed Monthly" and make Invisible. 3. Submit via email the users full name (First +Last) and Email and Phone Number and that comments field and the newsletter subscription checkbox result. 4. In the near future we'll want to also submit that email newsletter signup to another site (a service for managing newsletter subscriptions) so please design it so that'll be easy to implement. 8/28/2012 10:30pm EST. Updated the Attached files. You will notice that there is some conditional logic that reads a Parameter from the URL (?Country=US|UK|AU" Make sure you test all three countries it when completed. Update: Someone asked if we'd need 4 buttons for each country. Answer:We need 4 buttons. But the "post" info will change depending on the country value in the url Parameters. The aspx code changes the values of the current drop down list depending on Country= value. If no ?Country= parameter is set, it should default to Country=US. 2. I would like to avoid using Javascript if we can. 3.
