Create iPhone Application and Android Application - redesigned from a website

I am currently looking to use Ionic as a development tool as it allows me to push both an Android and iOS file out.
There is a form on the website below. I essentially need to recreate that form in an iOS app that will allow the form to send out to an email address with the designated subject Online Order for proper filing.
I also need a homepage where users can log-in with their Facebook accounts or whatever and have their addresses saved for future purposes.
I know it is a simple project but I'm just not sure how to do it alone (I am more of a graphic designer than an app developer).
Essentially, I would need the login page, a jump page that had an embedded instagram feed, a page to display my cookies, and an online form. Currently, I have been working on putting them in a container and then using the webView option in xCode to just work on it as I am a web developer. I hate how it is looking though.

I do not want someone to just do a webView of this application - I want the current web application converted and redesigned into a native iOS and Android app!
I need a pretty version of the link above - basically that has a place for users to sign-in.
