App for Severance Payment Calculation -- 2

In essence, the app calculates the corresponding severance payment for an employee who is terminated from employment. The total is based on a formula which takes into consideration the total amount of years worked, the highest hourly rate/salary in the past 3 year, and the highest number of regular hours worked in the last 3 months. Accordingly, the app needs a box to enter the date of hire, a box to enter the date of termination. Then it needs to have 2 parallel scenarios: hourly employees and salaried employees. Hourlies would need to fill in boxes corresponding to highest regular rate in last 3 years and highest amount of worked hours in last 3 months. Salaried employees would just need to place highest monthly salary in the last 3 years. Then a box would give the total severance amount. I would like the app to look like a clean sheet of a legal pad and when the amounts are entered to look as if they were being written in pencil, with the corresponding box dissapearing. The app would need to include some legal disclaimers, and some explanations for each corresponding box. I would also provide the applicable formula for the corresponding calculations. I want quality, cleanliness and simpleness.
