Simple Cordova/Phonegap file writing app

Im am in need of a very simple cordova/html5 app that will take 3 inputs from an HTML form built into the app and append them to a csv file which is stored on the sdcard. The form inputs are name (single line text field) model (single line text field) active (dropdown, Y or N labels, "Y" "N" values) They should append to a file called "[login to view URL]" on the sdcard. The fields are separated by commas. One record per line. There are no specific UI requrements, it can be as simple as an app with 3 inputs as noted and a submit button. No error checking is necessary.
