Thermochromatic Plastic square backing plate for mobile phones

I require plastic squares per these specs: Material - a thermochromatic pigmented plastic which becomes red when hot and blue when cold, it should be evenly colored 1) Square width and breadth is 45mm by 45mm, depth/thickness is 3mm 2) One 45x45mm side is flat, the other is rounded at the corners 3) The flat side needs to be adhesize such that it could stick on metal or plastic very securely and durably 4) The side with rounded edges should be smooth, and in the center of this side there should be a 1mm deep indent of 17mm by 17mm dimensions, with rounded edges.
