ASP.net Convert 7 PDF Forms to Online HTML - repost

We have a website that allows users to go online and fill out forms, some of these are in HTML and some in PDF format. We have 7 forms that are currently in PDF form that we need to convert to html versions. For the most part the forms are all HTML, but there is a little bit of c#.net and jQuery to handle data and calculations. I've attached all 7 PDF forms here which show the fields needed, and also the required calculations. 4 of these forms are very close duplicates, so there are really only 5 different forms to create. We need to make the style of the HTML forms match the existing HTML forms on the website. The previous forms were all built in JotForm, then exported as HTML. If you are interested in the project I can send login information to show you the style of the existing forms on the website. Only one of the forms require any jQuery calculations. There are mathematical calculations, giving totals of previous entries. This form is 'replacements-and-capital-improvements'.
