Build a landing page for my Adword

Hi Greetings. I have reviewed your website and can have it redesigned by my designer. There are some minor changes to be made to the text and images. Also when you click on the Free Trial button there seems to be no text fields like what to enter in those 2 fields before hitting the try it for free button, those field names also to be entered for example: name, email address or whichever you have selected for this signup page.
