Create HTML/Ajax Form

I would like someone to create the attached form. The form should allow to add additional vehicles and additional staff by clicking on the down arrow. On click the fields in the section will be duplicated as necessary. The form should not have any limit on the vehicles and staff that can be added. The form should include validation and submit into a database utilising Ajaz techniques. A PHP submission cannot be used due to the CMS the form is being integrated into. Once the form is submitted all data should be submitted into a database (2 tables?) and also all details emailed to a specified email address. No access will be given to the CMS so once the form has been tested on your own servers, it will need to be provided as HTML with a SQL query for the relevant tables and instructions on how to implement it. The form should be laid out as per the attached - however font styling will be applied by us on integration into the CMS. This is an immediate requirement.
