Redesign my PDF form, make it fillable and add some functionality

I have a medical form that I need converted into a fillable PDF. Some advanced function required (cascading dropdown, check boxes, esign, email button, save button).
Need nice clean design. It will be used on Ipad and computer so Acroform is best, even thought livecycle is better (unless you can tell me how to fill a livecycle form on iPad).
