I have a word document that I would like converted into a fillable pdf form. This form will be uploaded onto my website. Patients will fill in the form on the website, print, and sign it. The form needs to be saveable by the individual but not saved on the website. I have included an "example" of the layout that I would like. The word document (medical dental history) is my content. The PDF is the design example I would like to follow although I am happy to review your design suggestions. I would like to see examples of your past designs I would like to see drafts of the design and test it on my website before final approval
