We want to re-create our competitions booking form. The pages will have the same look and feel as the home page you made, but the form will be identical with a few small adjustments.

Page 1 of Booking Form:

Please gray out all the Sunday dates on the calendar.

Change “$10 OFF*” to “$20 OFF”

1 Blind Layer: A red bar across the top for error messages

Page 2 of Booking Form:

Change text to: We carry, recycle, donate and safely dispose of:

• Household Materials (appliances, furniture, electronics, mattresses, carpet and more)

• Office Items (computers, printers, servers, desks, boardroom tables and more)

• General Waste (drywall, brick, clean fill, yard waste and more)

1 Blind Layer for when viewer clicks “Price Estimator”

“Price Estimator” button changes to “Close”

Change text to:

“Our prices are based on volume only, whether it takes us 2 minutes or 2 hours to remove your stuff.”

“Hover over our load sizes to picture what your estimated removal price is.”

Please remove the “7/8” fraction.

“Please note this online tool is meant to help estimate your volume. Our on-site crew will provide a no-obligation quote for the removal. Taxes are not included.”

You do not have to add pictures of the furniture or truck.

This form will be the same. Please create separate blind layers for when “I am booking for a business” is checked off, and “I have a promo code” is checked off.
