Design a STUNNING stationary template & invoice/order form

The desired colors would be pink and white with black or something similar. I need an expert freelancer who knows how to design and make an online form. My website is [login to view URL] I need an outstanding stationary template that I am able to use to write letters and notes to my customers. The template must have letterhead of the store name, contact info at the bottom footer, and a whole boarder around the page. For the invoice/order form, I will attach a picture of one as an example and I can give you the text for each field and a different design, it has to match the letterhead so everything is one identity. I want my customers to be able to fill out the form on the computer and it be emailed to my work email and I can print it out. I will be also taking offline orders so the form must be able to print out and be filled by hand also. I found most of these image on fiverr and the average cost for the designs are $30-$60 but they have too many projects ahead of me and i need it done asap. I also would like for everything to match so I only want to work with one freelancer. Please read this over very carefully and message me if you have any questions. Please also look at all the files I attached for a more clear understanding of what I am looking for. I am ONLY looking for a template I can use to write to my customers and an invoice/order form. I do not need the other things associated in a stationary package.
