arpReach subscribe form integration into Thrive Themes

I'm using arpReach autoresponders and broadcasts but cannot integrate an unsubscribe form, to unsubscribe contacts, which is extremely basic stuff :-) but I cannot get it done. It could be done in minutes, I'm sure. Furthermore, I'd like to get rid of an html line that comes with all emails sent.
