Create checklist table with editable pdf cells

This is a very simple project that I do not have the time to do myself. I need a home inventory checklist created. It will be simply be creating a table, and entering the data that I will provide, into only one of the columns. It will look something like this: | LIVING ROOM | | Item | Serial Number | Purchase Date | Current Value | | Sofa | | | | | Couch/Loveseat | | | | | Coffee Table | | | | | Television | | | | | Other | | | | The resulting document will be converted to an editable pdf, which will allow the end user to type in their information into the other blank cells, then save and or print the document. I will also provide you with samples.
