Convert C# form to a iis6 web application

I have a very simple working c# 1-page form app that I need to turn into a web app that plugs into IIS 6 running on Windows 2003 server. It should be .net 2.0 compatible. I have provide a screenshot and source code. The only differenct is I want to pass 1 variable into the web app via Internet explorer to IIS and the app will lookup via an .inf file or something on the server-side and pass those variables you see in the screen shot in. I will provide the sample .inf file. Please respond with the bid price and what technology is best to use on the server side. Deliverables are: 1) Source w/ comments 2) Binaries that I can plug into iis 6.0 3) A 1 page sample html that allows me to click on a graphic button and it does a submit to the server and the graphic button changes color when it is successful. 4) Documentation on any server configs I need to do
