if you just need a data form, you should thinks about a simpler solution. I can give you a pdf file that include all components (text boxes, combo boxes, check-boxes) which you require. by default, it is a pdf file. this solution, I can totally help you. I hope this solution meets your requirements. also if you want to write software, more difficult and more expensive.
