In regard to your form revisions, I submit that I am the one for this project. I am a professional production specialist and have 20+ years of experience in graphics, especially in projects going to print. And, I have been an InDesign user since the very first version over 13 years ago. I am very adept at using InDesign's table feature, which is best for creating forms.
I also have extensive experience in Photoshop, Illustrator and output of final PDFs for print. I can be a one stop shop for all of your form / print needs.
Process: 1) receive the form and examine for an understanding of the required changes, as well as receive the InDesign files, if they exist; 2) If they exist, begin making the changes as requested, suggesting changes that would make the form more useful to your clients; 3) complete and supply a PDF for proofing/approval; 4) receive comments in regard to changes or approval and resubmit a new PDF if necessary; 5) supply a final PDF in the requested format and supply the packaged source files.
I use the Creative Cloud subscription, which is the latest version of Adobe's products. It comes with an ability to access all of Adobe's fonts. I also have a very large collection of fonts.
I can save the final file down to InDesign 4, which can be opened in any version above 4.
